Contemporary Education Foundation Application Form for the Data Owner
In order for your request within the scope of Personal Data Protection Law to be fulfilled, please complete and print the below application form and submit to the Foundation in person.
You may submit the completed and signed original of the form in person together with your identification to Çagdaş Eğitim Vakfı Kocatepe Mah. Feridiye Cad. No 53 Taksim/Istanbul, or alternatively use Notary or different channels to send the form as outlined in the Personal Data Protection Law, or can send the form with the secure electronic signature.
We will reply your request in a short period, maximum within 30 days free of charge. We reserve the righ to request additional charges in line with the tariff outlined by the foundation’s Personal Data Protection Board, should there be additional cost. We will contact you in order to clarify or complete, should there be missing information or document in your application.

This aplication form has been prepared in order to respond to your application in a correct and legal time frame by determining your relationship with the Foundation and, if any, fully identifying your personal data processed by the Foundation. The Foundation reserves the right to request additional document(s), information( birth certificate,or driver’s licence etc) for identification or authorization in order to eliminate the legal risks that may arise from illegal and unfair data sharing and especially to ensure the security of your personal data.
In the event that the information regarding your request are not accurate and up to date or the application is unauthorized, the Foundation does not accept any liability for such false information or requests arising from unauthorized applications.All liability arising from misleading or incorrect application lies with you.
